Hazel Irene Forkum, 84, of Paragould, died Sunday at Arkansas Methodist Medical Center in Paragould. She was born to the late Henry and Eva Forkum on July 13, 1930, in Greene County. She was also preceded in death by her husband, Dub Forkum; brothers, Richard, Jack, and Bobby Forkum; and sister, Rhoda Mae Greer. Hazel is survived by her brothers, Bill (Wanda) Forkum, Tab (Betty) Forkum, and Windy Forkum, all of Walcott; sister-in-law, Jan Forkum of Columbus, MS; several nephews and nieces; and her Belle Meade family.
